Job Description
Overview:
We are seeking Nurse Practitioners (NP) and Physician Assistants (PA) to conduct initial health assessments (IHA) for Medicaid members in Georgia. The program goes live October 1st, and providers will play a key role in helping members receive timely, in-home care.
This opportunity is flexible—providers can commit a few days per week, evenings, or weekends, or work full-time to complete assessments quickly. Ideal candidates will have a background in home health and be comfortable traveling within a 50-minute radius of their home.
Responsibilities:
- Conduct comprehensive histories and physical exams.
- Evaluate social determinants of health.
- Deliver health education on disease prevention, medication use, lifestyle changes, and self-care.
- Order diagnostics, develop and adjust treatment plans, and manage medications (as applicable).
- Complete and submit documentation within 24 hours of visit (primarily completed during the visit).

Qualifications:
- Active NP or PA license.
- Minimum 1 year of experience.
- Ability to complete 30 assessments within 1 month.
