Job Description

Blize Healthcare is looking for a Licensed Vocational Nurse to join our team. You will provide skilled nursing care to patients in their homes, often under the direction of a registered nurse or physician. You will perform a range of duties, including patient assessment, medication administration, wound care, and patient education, while also coordinating with other healthcare professionals and family members.

Location: Contra Costa and Alameda.

Job Description:

• Assessing patients' needs, developing and implementing care plans, and monitoring patient progress.
• Administering medications, including injections and IV fluids, as prescribed by a physician.
• Providing wound care, including dressing changes and monitoring for infection.
• Educating patients and their families about their health conditions, medications, and care plans.
• Documenting patient assessments, care plans, and progress notes in accordance with agency policies.
• Observing patients for changes in their condition and reporting any concerns to the supervising nurse or physician.

Required Skills & Qualifications:

• One year of nursing experience.
• Home Health or Hospice experience strongly preferred.
• Basic nursing skills, including medication administration, wound care, and vital sign measurement.
• Ability to connect with patients and their families on an emotional level.
• Ability to identify and resolve patient problems in a timely manner.
• Physical strength and ability to lift up to 50 pounds, as needed.
