This program helps individuals with serious mental health needs receive treatment rather than incarceration, supporting long-term stability and reintegration into outpatient care.
Our care teams include therapists, case managers, prescribers, and nurses who meet patients in the field and work collaboratively over months to years. After orientation, physicians typically complete field rounds with a partner or team, and documentation can be completed remotely via secure, web-based clinical tools.
