Position : Care Manager
Location: Kent County, Delaware (Hybrid.)
Duration: Contract to Hire Role
- Active DE RN license with case management experience.
- Conduct assessments and develop care/service plans.
- Collaborate with PCPs, therapists, behavioral health providers, and others.
- Order DME and coordinate member discharges from facilities.
- Educate members on social determinants and available resources.
- Flexible work hours to meet member needs.
- Proficient in Word, Excel, Outlook, and electronic documentation systems.
- Reliable daily transportation for field visits (2–3 times/week).
- Experience with geriatrics, behavioral health, and home health preferred.
- Understanding of cultural competency in care delivery.
- Meet regulatory deadlines and maintain cost neutrality.
- Conduct phone and face-to-face assessments in homes or community settings.
- Must have a dedicated home office space and comply with telecommute policies.
