Travel MDS Nurse (RN or LVN) – Hybrid Remote | Central CA & SF Bay Area

Are you an experienced MDS Coordinator looking for flexibility, variety, and a chance to make a real impact?

Evergreen Healthcare Group is hiring a Travel MDS Nurse (RN or LVN) to support skilled nursing facilities across Central California and the San Francisco Bay Area in a 50% remote / 50% on-site hybrid role.

What You’ll Do:

  • Support clinical teams with timely and accurate MDS assessments
  • Coordinate ARDs and IDT participation across multiple facilities
  • Conduct on-site visits for training, audits, and assessment support
  • Utilize EMR systems to complete MDS documentation and ensure regulatory compliance
  • Promote quality outcomes through collaboration with facility leadership
  • Step in to support urgent MDS coverage needs at facilities experiencing staffing gaps, including temporarily filling in during transitions until a permanent replacement is hired.

Requirements:

  • Current RN or LVN license in California (unencumbered)
  • 4+ years as an MDS Coordinator in a skilled nursing facility
  • Strong understanding of the RAI process and care planning
  • Comfortable with travel across Central CA & SF Bay Area
  • Proficient in EMR systems (PointClickCare preferred)

Preferred:

  • RAC-CT Certification
  • Medicare utilization review experience
  • Strong documentation and analytical skills
