General Purpose of Job: Care Coordinator/Community Health Worker (CHW) plans and coordinates all activities related to the Early Childhood Development and help patients overcome barriers that prevent them from getting the care they need. Care Coordinator/Community Health Worker serves as liaison between interdisciplinary Care Teams (including Patient Navigators, Care Coordinators, primary care physicians, and additional health care providers) and expedite client service referrals. The Care Coordinator/Community Health Worker will be located within their practice setting and will be responsible for planning, delivery and evaluation of case management services. Care Coordinator/Community Health Worker participates in policy development, community resource management and evaluation of patient care and program. Care Coordinator/Community Health Worker will be responsible to provide social determinant screenings and refer the patients to the provider as needed. Care Coordinator/Community Health Worker will supervise the Patient Navigator/CHW’s from ECD and NIH grant.
